PROCEDURE 1.) Persons requesting equipment will contact the Highland Park Operation Weed and Seed Headquarters (313) 868-5136 to check for availability and schedule it's use. A brief description of the event, type of equipment needed, and how long it will be needed for the project is needed in writing. Neighborhood association members requesting equipment will need written confirmation from the association president or another association officer that the equipment is being requested by the association for an association sponsored project. 2.) When the request is approved, the persons requesting the equipment will receive and sign a copy of the equipment policy and procedure and sign for the equipment at the Weed and Seed Headquarters. The storage areas for the equipment will be contacted and informed of who will be picking up the equipment and at what date and time, A release form indicating approval to release specific equipment will be issued and should be taken to the storage area to pick up the desired equipment. 3.) All clean-up equipment will be stored at the Weed and Seed Headquarters. Jerome Drain or his designee will be the contact person.. 4.) The release form should be signed by the person releasing the equipment. 5.) All equipment should be initially returned to the Highland Park Operation Weed and Seed Headquarters so its condition can be verified. Any reports of damaged or missing equipment will be made at that time, Operation Weed and Seed will then return equipment to it's assigned storage area, I HAVE READ THESE POLICIES AND PROCEDURES, AND UNDERSTAND MY RESPONSIBILITIES FOR USE OF WEED AND SEED COMMUNITY PROJECT EQUIPMENT.
